SISIETY WAIVER
All girls MUST have a waiver to participate:
Each individual participant (or authorized guardian, if minor) must read and agree to this release of liability prior to participating in any SISIETY Girls Club (SGC) You will give your daughter permission to participate in SGC activities and agree to these terms by checking the box that says "I agree to the terms & conditions"
Product Usage:
By signing this permission slip, you acknowledge your understanding of SGC activities which may include the use of beauty and skin care products. SGC is not responsible for any injuries or allergic reactions that happen as a result of using products during activities.
Photos:
Participants may have their photos taken during SGC events. Photos may be uploaded to a private online gallery and/or social media sites and shared amongst guests and their friends. SGC is not responsible for how photos are shared. By signing this form you are acknowledging that photos and videos may be taken of your child. You acknowledge that SGC is not responsible for how photos are shared.
Accidents:
SGC is not responsible for any damages, accidents or injuries that may incur while under our supervision inside our studio location or property surrounding SGC.
Age Allowance:
The youngest age we serve is 9. Girls younger than 9 will not be able to participate in SGC activities.
Release of Liability:
In order to participate in SGC events or activities, I, the undersigned, freely assume all risks, both known and unknown, and assume full responsibility for my and my child's participation and safety. I, for myself, and on behalf of my heirs, assigns, personal representatives and next of kin hereby release and hold harmless SGC their officers, officials, agents or employees, from any and all liability for injury, loss or damage to personal property. I acknowledge, understand and agree that I have read this release of liability and assume all risk associated with participating and that I sign this release of liability voluntarily and without inducement.
